This is where most of our referrals arrive. Nirvana's profile is listed with our current services and open counties, so you can send it to us there directly.

Your county board's own system

Some boards run their own referral or provider matching process rather than using LiveCare. If yours does, send it through there and it reaches us the same way.

For families and individuals

I am looking for support for a family member. Where do I start?

Start with your county board of developmental disabilities and ask to speak with a Support Service Administrator. The SSA confirms eligibility, builds the individual service plan and sends referrals to providers like us.

If you are not sure which board covers you, the county board links further down this page will get you there. You are welcome to email us too and we will point you in the right direction, even if the answer is not Nirvana.

Can a family member be hired as the paid caregiver?

Often yes, through Participant-Directed HPC. A person may hire their own family members, provided that relative is not also designated to help with employer functions.

Two limits worth knowing early. Parents cannot provide this service to their own child under 18. And who acts as the employer depends on the waiver: on Individual Options and Level One you are the employer directly, while on the SELF waiver Nirvana can co-employ under an Agency with Choice arrangement.

Raise it with your SSA early and we will work through the detail with you rather than guess. The governing rule is Ohio Administrative Code 5123-9-32.

What does this cost us?

Authorized waiver services are billed to the funding source attached to the individual service plan, such as the I/O, Level 1 or SELF waiver, or local county board funds. Families are not billed directly by Nirvana for authorized services.

What is the difference between HPC and Participant-Directed HPC?

With standard HPC, Nirvana recruits, hires and manages the staff. With participant-directed HPC, you choose the person and set the schedule, and Nirvana carries the employer responsibilities including payroll, background checks, training records and compliance.

Do you serve my area?

We serve Southwest Ohio, and that is where we can usually respond fastest because it is where our staff live.

Our certification reaches further across the state than our day to day service area does, so please ask rather than assuming the answer is no. Tell us where you are and we will give you a straight answer either way.

How do I send a referral, and how fast will you answer?

Through LiveCare Marketplace, or through your county board's own referral process if it uses one. Referrals cannot be submitted through this website, and we would rather be upfront about that than have one sit unread in a form.

We respond within one business day with a clear yes or no. If we decline, you get the actual reason rather than a vague note, so you can move the referral along quickly.

Can you really staff this, or will it sit open?

Fair question, and the honest answer is that it depends on the placement. We are a newer agency without a large bench of idle staff, so we assess capacity before accepting rather than after.

If a placement needs specific hours, a specific gender of staff, or a start date far out, we will tell you what that realistically means for our timeline instead of accepting and going quiet.

Which services are you currently accepting?

We are accepting referrals for Homemaker Personal Care, Participant-Directed HPC and HPC Transportation. We are also certified for Community Transition, which reimburses one-time household start-up costs on the Individual Options waiver.

We are certified for Non-Medical Transportation and Shared Living but are not accepting new referrals for either at this time.

Do you take participant-directed and family caregiver arrangements?

Yes, and we actively want them. These placements tend to work well for everyone because the caregiver already knows the individual and the routine. Send them our way.

How do I apply?

Use the application form on the careers page. It takes about two minutes, works on a phone, and needs no account or login.

You do not need to attach anything to apply. Email your resume and certifications to NirvanaServicesLLC@yahoo.com whenever it is convenient, or wait for our reply and we will send you an upload link.

What qualifications do I need?

To be considered we need your driver's license front and back, the DODD eight-hour orientation certificate, current CPR and First Aid, a Social Security card for I-9 verification, and a high school diploma or equivalent.

BCI and WebCheck fingerprinting and a BMV abstract are needed before a placement starts, but they do not have to be done before you contact us.

What does it pay?

Direct Support Professional roles pay $17 to $20 an hour. Where you land in that range depends on your experience, your certifications and the county. Mileage is reimbursed when you drive for work.

We will confirm the exact rate for a specific opening before you commit to anything.

How quickly will I get hours?

Honestly, it depends on referrals we have been awarded. We will not tell you a shift is yours before the contract is in hand. If something is a possibility rather than a certainty, we will say so plainly.
